contractual terms

Should Pastor Tommye Armstrong, aka, “Speaker” accept your request, Enter your name aka “Host” of Enter name of organization/group/affiliate aka “Affiliate” or on the behalf of yourself aka Self agree to the following:

 

·         The Speaker  requires a non-refundable 50% deposit of honorarium ($3000 per event out of state, Speaker’s local state $1000; over 60 miles, and within Speaker’s local city area $400 under 60 miles) via certified check/money order within two weeks of your approval email date.  If the deposit is not received, your event date will not be confirmed, the date will be reopened for other requests and a new request will need to be resubmitted for approval. (Note: PayPal option is available upon request/Website or $THOLM21)

 

·        The Speaker  requires a $25 per diem (if meals are not being provided: $40 per night) which can be given in cash or in addition to honorarium fee.

 

·        Honorarium fee covers up to 2 days (if event runs over to next day) and any additional days honorarium fee applies daily within an extended event.

 

·         Host will email a complete agenda of event from flight landing to departure at least 48 hours prior to Speaker’s arrival (out of State).

 

·         Flight arrangements are to be completed by Host or Self.   The Speaker will approve ALL flight itineraries PRIOR to being purchased.  Once approved, the Speaker will provide further instructions as needed or by “host/self” request. (out of State)

 

·         The Speaker  reserves the right to cancel any previously made commitments and Host/Affiliate/Self forfeits claims to any honorariums previously paid if flight is purchased prior to the Speaker’s review.  (Note:  Window row seating is preferable followed by aisle seating, business/first class is preferred but is not required)

 

·         Host will arrange ALL ground transportation.  Mandatory ground transportation is to be provided to/from airport, venues, events and group outings including but not limited to sights and meal locations.  Speaker chooses not to drive.

 

·         Host will arrange overnight accommodations providing an email confirmation with all charges being billed directly to Host or AffiliateWhere the Speaker is staying is to remain confidential.  (Note:  Hotel must have fitness center, room must include internet access, iron/board, bathtub, refrigerator, desk, and microwave.)

 

·         Speaker  reserves his right to cancel participation if participants are added to event without his knowledge or if ticket prices are increased or required after previously indicating they would not be.

 

·        The Speaker  reserves his right to cancel participation if there is any breach to this agreement.

 

·        Host is required to video or record  event and provide Speaker with a copy.

 

·         All flyers/posters must be approved by the Speaker prior to dissemination and must be available at least 2 weeks prior to event and must include Speaker’s website and titles; etc. If necessary, the Speaker shall receive complimentary registration for himself as well as family or personal assistant to the entire program.

 

·         ALL modifications to event must be approved by Speaker prior to being made. Any change, addition or deletion to this agreement must be submitted in writing to all parties concerned immediately.   All changes will require verification.

 

·         The Speaker reserves the right to cancel participation if there is any breach to this agreement. To print form and contractual information please click here
